For each unresolved issue, record the question, the person supplying evidence, the qualified reviewer and the decision affected. If two answers conflict, keep both dated versions visible and ask the reviewer to reconcile them; do not quietly choose the convenient answer. Send one agreed update to the participants so a changed assumption reaches everyone relying on it. The log coordinates information; it does not establish signing authority.
